You need your passport but try to only give them a copy not the passport.
Money.
Health Insurance for when that truck hits you.
A helmet insist they give you one.
Balls to drive there.
An international drivers license, or Thai license is the normal requirement by law.
The owner of the motorbike will not usually ask to see your license.
They will need to see your passport and in some cases they will want to keep your passport as collateral or ask you to pay around 3,000 baht security deposit.
So in fact they might hire to a foreigner who does not have a license.
No I'm not saying you can break the law. You will be fined 400 baht
for driving without a license.
The police do regular checks with roadblocks or just magically appear from nowhere. If caught you will be asked to leave the motor bike with the policeman, take alternative transport to the police station, pay the fine then return to the scene to show the policeman proof of payment before you can get your motorbike back.

A small motorbike, scooter, is easy to maneuver in the traffic that can become congested especially in Phuket Town.
In almost every street in the accommodation areas of Phuket you will be able to rent a motorbike or scooter. The rates of rental start at around 150 - 200 baht per day for a late model 125 cc Honda, Yamaha or Suzuki.
Don't be afraid to negotiate if you are renting a motorbike for one week or one month. I have seen monthly motorbike rental rates advertised for 2,700 baht per month.
Many of the hotels or guest houses have motorbikes, scooters for hire so you might be able to make a deal with the guesthouse where you stay. They may not require you to hand over your passport, if that bothers you.
Don't expect Thai drivers to be driving with the same respect for the driving rules as most drivers in your country do. There are many road accidents every week in Thailand. Some foreigners die in these accidents. Eighty percent of these fatalities have been hirers of big motorbikes. Drive with care and you will survive.
When riding don't be bothered by the honking horns. It just means beware there is a vehicle approaching. Usually it is a light tap of the horn and not a long aggressive honk like we give in the west.
I lived in Phuket for 18 months. Renting a scooter in Phuket is a good idea. It gives you freedom to travel around the beautiful island. I say go do it. Drive safely and enjoy.
Most rental shops will only ask for a passport and could care less if you have a license, it's a good idea to make a copy of your passport to leave instead.
Is it a bad idea? Foreign tourists account for about 60% of all motorbike accidents in Phuket. Many die or are seriously injured, but the vast majority of tourists rent motorbikes without incident. It's only a bad idea if you have an accident.
If you're careful, wear a helmet and don't drive drunk it can be a great way to get around and see Phuket. If you're an inexperienced rider it's probably not a good place to learn. If you do decide to rent a motorbike, look for a bike with an automatic transmission to make riding a lot easier.
all u need = $ & passport. u dun even need a driver's lisence. i hv been there a few times & rented bike & car b4. their bike & car r old... but stil can work. anyway, it's v convenience if u can drive urself as they do not hv taxi all the time. but, it's kinda danger if u dun hv any bike's experience as there's serious accidents b4. n insurance DO NOT cover 4 that.
You need a passport to leave with the hirer and about 200baht - traffic is not too bad but heavy in early evenings. You dont say where you are from but traffic drives on the left, ( mostly but you can meet motorbikes using your side of the road as they use shortcuts !!)
